We accept Medicare and most major insurance plans with out of network benefits including:

· AETNA

· Blue Cross and Blue Shield (BCBS)

· CIGNA

· Horizon BCBS

· Health Net

· Oxford

· PHCS

· United Healthcare

and many other insurance plans.

We also accept No-Fault and Workers Compensation cases.
 

We do not accept Medicaid and HMO policies.

Insurance Questions:

Sometimes we cannot give an immediate answer to the question "Do you take MY insurance?" without speaking to the insurance carrier directly.  We do not want to mislead you or give you incorrect information.

 Prior to your first visit we will contact your insurance to ascertain your particular insurance coverage and/or requirements. We will advise you of this information before you start physical therapy
